Our highly trained staff can help you make the best choices for your needs while providing you with friendly service and expert advice. We offer solutions to all of your home health care needs. We work with you, your family, and any other medical advisors to design a Plan of Care specific to you. Maintaining and improving your quality of life is one of our priorities.
Step 1: Assessment
Our RN Care Manager will meet with you and conduct an evaluation of your current status. This will include health issues, injuries, other available support, activities of daily living, and any other issues critical to your continued safety and well-being.
Step 2: Care Plan
Based upon a careful analysis of your situation, we will develop a custom Plan of Care. Your Plan of Care will be coordinated with any other health care advisors you may be working with. Additionally, we will communicate with any family members or friends you would like to have involved in your care.
Step 3: Care Giver Orientation(s)
Your caregiver(s) (Nurses, CNAs, HHAs, etc.) will be carefully selected based upon skills, personality, availability, and your continued input. You and your care team (your family members, case managers, and any one who is part of your care) will be able to meet and talk with your caregiver during orientation so that you can build a strong working relationship.
Step 4: Quality Control & Monitoring
Our patients benefit from regular visits that include:
- Weekly Assessments
- Monthly Staff Meetings
- An updated Plan of Care as needed
- Monthly Updates sent to health professionals as directed
