Patient Scenario #1 - Convenience
This patient scenario involved a 23 year old man involved in a motorcycle accident. His injuries included a shattered leg along with both wrists broken.
He was discharged from hospital without home care orders or a payer source resolved. His mom, a nurse, was unable to continue providing care without risking her employment. She was told she needed to come back to work to risk losing her job.
The family contacted multiple providers, but no one would take them on without confirmed payer source information.
AdvisaCare received the initial call on Labor Day and began providing care the following Tuesday morning. With our expertise, we were able to commit to providing care without pre-authorization so Mom would not lose her job. Within just a few weeks we had figured out the payer source in order to continue home care and therapy.
We provided care and transportation for this patient during his 6 month road to recovery.
Patient Scenario #2 - Reliability
This patient scenario involved a 29 year old woman who was 2 years post Spinal Cord and Brain Injury including partial paralysis on left side. We received the referral from a rehabilitation center specializing in brain injuries.
The family had used 4 agencies that (in their opinion) had over promised and under delivered. This resulted in Mom & Dad providing 24 hour Care / Supervision, while trying to work full-time, since they lacked trust and confidence in agencies.
As with many major brain injuries, this patient would 'act out' when she does not trust caregivers but biting, raising voice, etc... New caregivers were showing up to the home without the family being notified and with no orientation.
AdvisaCare took over care in 2009. We orientate all caregivers on a 12 hour shift and have continued the commitment and care that was promised. Our managers visit the family weekly to be sure that our promise is being delivered.
The Case Manager has noticed a remarkable change in the patient and that the family is now more relaxed.
We have been providing care for the family for over 2 years.
Patient Scenario #3 - Quality
This patient scenario involved a 53 year old man with quadriplegia from a vehicle rollover accident. The patient went through 6 agencies in 2 1/2 years. This last agency gave them a 48 hour notice to terminate the service.
AdvisaCare came in and got the patient involved in out-patient therapy with RIM. Our managers visit with the family weekly to help this family manage their new life.
Sadly, this patient's spouse passed away unexpectedly. The Owner & Branch Manager made a point to meet the Sheriff at the home so the patient wouldn't receive the news alone. AdvisaCare was there to provide support to our client in his time of need, outside the scope of his own personal care plan.
This client has been with us for over 4 years now.
Patient Scenario #4 - Flexibility
This patient scenario involved a woman with paraplegia who was 18 years post injury whose son at the time was 8 years old. Patient was a single mom and had son staying with family during her hospital stay. She had been in hospital 4 weeks with a state 3 wound.
AdvisaCare received the referral from the Insurance Company on the Thursday before the Adjuster left on vacation until the following Tuesday for the holiday.
The patient decided on Saturday (of Labor Day weekend) that she wanted to get home before son started first day of school. Because the hospital director would not come to handle the discharge on the weekend the patient had to wait an extra day to be discharged. She was discharged on Monday (Labor Day).
We set up lodging for out-of-town caregivers until local experienced caregivers could be found. We packed up her hospital room, provided groceries for a cookout and brought her home on Labor Day.
We have been providing this patient with care for over 2 years.
